This form is a binding contract so be sure to fill it out exactly as you want.
You are filling out this form to either approve of a recent website proof you have been contacted with or you are offering changes to the proof. Please be as specific as possible when offering changes or alterations to the design proof.
This is important to ensure that time is not wasted on determining the meaning of the information you provide and ensures mis-interpretation is kept to a minimum.

Payment Info
Most payment schedules, unless previously agreed upon, require the first phase, (all work completed up to approval of design proof) to be paid at the time of design approval.
If you recieved an invoice for the first phase of your project and would like to submit payment online, you may do so here.
Paypal is the only online payment method accepted.
